HOW TO SIGN UP FOR A SPORT
Please complete the steps below to complete the sports sign-up process. Please note that the sign-up process must be completed each season by a parent/guardian.
Step 1: Sign up via online registration on the Genesis Parent Portal (NEW for 2024-25)
Step 2: Physical Forms:
Print the appropriate medical forms and submit to the Nurse's Office.
New Physical Form
90-Day Update Form - Health History Update form to be filled out.
90-Day Update Form Memo from State (Health History Update Memo with same form within Memo)
NOTICE: ACTIVITY FEE SUSPENDED FOR THE CURRENT SCHOOL YEAR.
While there may be individual fees associated with specific clubs or activities, there is no additional district level activity fee requirement for participation in any HMS sports or extracurricular activities for the current school year.
Step 3: Activity Participation Fee Registration Form
There is a $40 annual fee to participate in any
HMS extracurricular activities/sports or clubs.

Opioid Fact Sheet
For those students who make the team, this sheet is distributed twice a season. It helps the coaching staff monitor the academic and behavioral status of their players. Coaches will distribute the progress reports on the dates established by the athletic director. If you lose it, click on Progress Report above and print it out. Copies are also available in the main office.
